Child Care Authorization

Child Care Authorization is a little-used form that may be extremely beneficial in ensuring that your children receive immediate medical or other assistance in an emergency. We highly advise you to preserve health cards and other pertinent information with this Authorization to ensure that the Child Care Provider is completely prepared in the event of an emergency, which is not the time to be looking for medical permits or paperwork.
You should place this paper on the front hall table for the Provider’s convenience. You might always advise that they maintain a duplicate in their car or yours, if they utilize it.
You should always execute this paperwork if you are leaving home overnight, but much more so if you are leaving home for a lengthy period of time and have someone else care for your children.
- Make several copies and sign them. Give one or more to the Provider and retain them as previously indicated.
- This health care document can be really beneficial. As a practical matter, print several copies after filling in the blanks and keep them on hand for use on travels.

The purpose of this letter is to advise you of the authority given to _________________ (“Child Care Provider”) over _______________________________________ (“Minor Children”).
This grant of temporary authority shall begin on _____________ and end on __________, unless terminated earlier by any of the undersigned.
The above Child Care Provider shall have the absolute and final authority to:
1. Seek appropriate medical treatment or attention on behalf of the Minor Children as may be required by the circumstances, including but not limited to, medical doctor and/or hospital visits.
2. Authorize medical treatment or medical procedures in an emergency situation.
3. Make appropriate and necessary decisions regarding clothing, bodily nourishment, and shelter.
4. Explain absences from school; pick Minor Children from school.
